PAYMENT POLICY

Medical services are provided and billed by Rafik Aboul-Nasr M.D. Our provider participates in most all of the major health insurance plans in the region.

 

If you have insurance, we will be to file a claim on your behalf.

Please be aware that with any health plans, there may be deductibles and copayments for which you could be responsible.

Fees & Payments & Billing & Insurance

Our office makes every effort to keep down the cost of your medical care. We ask your cooperation in these efforts, and in following the policies we have established to help us to minimize billing costs in our office. Office visit charges are based on the complexity of evaluation required. Additional charges are made for laboratory tests, diagnostic tests, or other procedures.

Our payment requirements are consistent with our participation in each insurance plan. All co-payments and deductibles are due at the time of the patient visit. We accept cash, checks, credit cards (VISA, MasterCard,Discover). There is a $35 fee for returned checks.

If you provide us with current insurance cards containing valid policy and claim information at the time of your visit, we will directly bill the insurers with whom we have contractual agreements.

QUESTIONS ABOUT YOUR BILL

What billing or insurance information will I receive?

​You will receive a copy of your bill. After your insurance has paid, we'll provide you with a monthly statement of your account.

How long will it take to get things settled with the insurance company?

Generally, it takes from 45 to 60 days to obtain payment from an insurance carrier.​

​Why am I getting bills from physicians?

​Each physician who provides professional services will send you a separate bill from his or her office. These physicians will include:

- your own physician
- other physicians who conducted tests and procedures
- consulting physicians chosen by your primary physician to read and interpret test results.

These physicians may include specialists in anesthesiology, cardiology, emergency medicine, nuclear medicine, pathology, radiology, or urodynamics. You will be responsible for paying these bills as well.

​I received a notice that my insurance company has paid on my bill -- but I can't understand how they calculated their payment amount. Do you know?

​If we have received any such information from your insurance company, we'll be glad to share it with you. However, for answers to any questions about insurance payments, deductibles, or co-payments, you generally need to check with your insurance company.

School Forms, Day Care Forms, Physical Examination Forms, Disability Forms, School Medication Forms

Please allow 72 hours for completion of forms that are not presented at the time of an office visit. There is a $15 charge for the first page and $10 for each additional requested, that must be paid at the time the forms are presented to the receptionist. Completed forms must be picked up at our office.

School/Work Activity Status Notes and Excuses

Please ask the doctor or nurse practitioner during your office visit regarding recommendations for your activity status – regarding days they recommend you stay away from work or school, any activity modifications, and when you should return to work or school. This will be written on your itemized bill and the front desk will issue a written excuse or note at time of check out. If activity forms are not picked up at the appointment. Because of confidentiality issues and concerns we will not fax notes to jobs or schools.

Worker’s Compensation

Patients requesting appointments for evaluation and treatment due to injuries or problems related to Workers Compensation will be required to pay $150 before they are examined by the physicians.

Prescriptions & Refills

Routine prescription renewals: Please contact your pharmacy directly for refills before calling our office and they will utilize our fax-back service to renew your prescription. Please allow 72 hours for prescription refills. This allows the prescribing provider time to review your request and make the proper authorization.

Medical Referrals

All requests must be made at least 72 hours in advance of specialist appointments and should be picked up at this office. We cannot routinely fax referrals to specialists’ office.

Medical Records

Copies of medical records may be obtained with 7 days advance notice. Costs are established under direction of the Maryland Annotated Code, currently $22.88 plus $0.76/page plus postage/shipping costs.

Lab and Test Results

All normal lab and test results will be available within two weeks of the physicians’ receiving the results. Abnormal results or those of an urgent nature will be phoned directly to make a follow-up appointment as soon as the results are available. We will not notify you if your blood or lab results are normal. You can call and request a copy of your bloodwork and pick them up after 24 hours notice.
