Important Patient Resources for First Time Patients:

• Meet your Physician – As a first-time patient, we invite you to learn about our doctor here at our online web site.

• Locating Physician Office – see the Maps & Directions to see where our office is located.

• Knowing Your Insurance Options – Review our updated list of Participating Health Plans. We recommend you call your health insurance company to find out which services are covered by your individual health plan coverage and to learn about your health benefits before you arrive to see one of our providers.

• Accessing Wheelchairs – We do have a ramp infront of the office building with Handicap parking available.

• When to Arrive & What to Bring – On your first visit, please arrive 15-30 minutes early. Please bring your current insurance card to expedite your registration and be prepared to pay your co-payment, if applicable.

• Office Parking – For the convenience of our patients, we have 1 large lot available with parking directly infront of the main entrance to the office

• Downloading Forms Ahead of Time – Before your appointment and depending on the reason for your visit, please download form, fill-out and bring Registration Forms to your visit.
